MEDICAL REQUIREMENTS

(IMMUNIZATIONS/HEARING/VISION/TB TESTING/HEALTH STATEMENT)

The state requires that children have proper immunizations and that records be kept in the child's file at the center. Children will not be allowed to attend if these records are not up to date. If a child is exempt from any immunization, the proper documents must be provided with a corresponding statement.

To comply with state law, within one year of enrollment, it will be necessary for the parent or guardian to supply The Depot Learning Center with a written statement signed by the child's physician stating that the child is physically able to participate in the full program of The Depot Learning Center.

Vision and hearing tests are required to be done on children four years of age and up. You may have your child tested by a private physician and submit the results to The Depot office promptly.

TB Testing is not required in our area and so our staff members are not tested at this time. This is subject to change by the decision of the state health department and The Depot Learning Center will comply with that decision should it occur.
